New Richmond Curbside Check-In

Please fill out this form as completely and accurately as possible so we can get to know you and your pet(s) before your visit.

Clients are expected to wait in the parking lot while their pet is inside the clinic. There will be a fee assessed for any patient that must be held in our facility beyond the end of the appointment. The pet will be placed in a kennel until the client returns to pick up the pet as soon as possible. Pets left overnight without contacting the clinic may be turned over to animal control.
